General Patient Information
The physicians of Optimal Rehabilitation Associates specialize in the field of Physical Medicine and Rehabilitation, which is also known as Physiatry. We emphasize the diagnosis and treatment of disorders that may produce temporary or permanent physical disability and sometimes mental disability. We commonly treat people with the following conditions:
- Musculoskeletal pain
- Joint pain (including shoulder, elbow, knee, hip, etc…)
- Neck and back pain
- Tendonitis
- Sprain/strain injury
- Nerve injury (including carpal tunnel syndrome and radiculopathy)
- Neuropathy
- Work injury
- Fibromyalgia
- Stroke
- Traumatic brain injury
- Spinal cord injury
- Loss of limb (amputation)
We see patients of all ages and our goal is to reduce or relieve pain; restore functional ability or ability to walk again; and restore quality of life lost due to illness or injury.
We work closely with other rehabilitation professionals such as physical therapists, occupational therapists, speech therapists, rehabilitation nurses, psychologists, and social workers to provide the most comprehensive, quality care possible. Our goal is to return the people back to the community so they again can enjoy doing the things that they used to do.

To Schedule an Appointment
You may reach our office between the hours of 8:30 a.m. to 5:00 p.m. Monday through Friday at 713-861-2022.
While we make every effort to maintain an “on time” schedule, operation schedules do not always run smoothly and at times our patients can experience a wait for their appointment. If you are unable to keep an appointment, please notify our office as soon as possible.
When arriving for your appointment, please be prepared to show a valid picture identification card and insurance card so that we may make copies and or update your files. Also, notify the secretary of any changes in address/phone numbers since your last visit.

Physician Questions
Phone calls received during regular office hours are answered by one of the office staff. They will assist you in every way possible, and please provide them with adequate information so that they may take care of you as soon as possible. Each doctor has set times for returning patient calls during the day, and the office staff can instruct you as to when to expect a return call.
Urgent calls are handled promptly. In the event that you are having a life threatening emergency, please call 911 or proceed to the nearest emergency room.

Prescriptions and Refills
Prescriptions and refills are only handled during regular business hours. If you are in need of a refill, please request your pharmacy to fax the request to 713-861-2234. If it becomes necessary for you to contact our office, please be prepared to provide the following information: pharmacy name and phone, and medication (s) including strength and dosages.

Insurance and Disability Forms
A payment of $15 will be required for the physician to complete disability and other forms. Completed documents will not be returned to the patient until payment has been received in the office.

***Keep in mind that insurance contracts may change at any given time. It is the patient’s responsibility to verify that their provider is contracted with their plan. However, if your insurance carrier is not listed above, call our office at 713-861-2022 and we would be happy to verify your insurance coverage to see what your out of pocket expenses may be.***

Insurance, Fees, and Payments
Each time a patient visits our office they may be asked to verify current insurance coverage, sign a release of information, an assignment of benefits (authorizing your insurance carrier to pay us), and present your insurance cards.
Our office participates with Medicare, Traditional Medicaid, and most other insurance plans (see list above). REMINDER: If your insurance requires a referral to see a specialist, it is not our offices responsibility to obtain the referral. Referrals must be obtained prior to the visit. Please contact one of our office staff at 713-861-2022 in regards to insurance plans we participate with.
In effort to reduce costs to the patient, our office has entered in to contracts with insurance plans to provide services to you at a negotiated rate. However, your plan may require you to pay co-payments, deductibles, and or co insurances to our office. These fees are due at the time of service and may not be billed to you at a later date. Billing patients at a later date can be considered a violation of our contract with the insurance company, and therefore potentially subjecting our office to contracts being terminated with your insurance company.
In the event that you are self pay, or our physicians are considered non-contracted with your insurance company, we are very happy to provide services to you. However, fees are due at the time of service and our office will provide you with a receipt so that you may obtain reimbursement from your insurance carrier.

Procedure and Medication Fees
Procedure and medication charges such as Botox Injections (non-cosmetic only), are very costly to the clinic and may require our office to precertify through your insurance carrier. Our office will verify that these procedures are covered and what your potential out of pocket expense will be. Patient out of pocket expenses are required prior to services being rendered. We will file the charges to your insurance and accept assignment of benefits on your behalf. A period of 60 days will be allowed for your insurance to process and pay your claim. Once the 60 days has lapsed, and the insurance has not paid, we will apply the balance to your account and expect payment from you. We ask that you contact your insurance carrier about 45 days after the procedure to assist our office in claims payment. Ultimately, it is the patients responsibility, not the doctors office to ensure claims payment. However, we will exhaust all efforts to collect on your behalf.
